I can't find my finished form. HELP!

I have a form that I have placed on a website, but I may have created it on a different Jot account.  Could you look for this form (Leasing application) under the user account email Tiff10Properties@mail.com?

I need to scale the size of the form so that it fits the web page (I got help with this last week).

The form in that page was created under the username "Lharris123" and the email address for password reset purposes is "lavern.harrison@gmail.com".
