Collecting passport number compliance

I need assistance understanding the parameters for collecting our clients data on our uncruise tour member form.  I've getting notices that I'm not compliant, but need to understand what the issue is.  Can someone please call or email me specifics?

1) do I need to change my signature widget? Or make it not mandatory?

I could not find a signature widget in your form. Please add a signature widget to your form, make it required and you should be good to go.

I recommend you add the E-Signature and the Terms & Conditions widget explaining to the user that they agree to give sensitive data such as Passport Number.
