Why is our google spreadsheet integration not working?

I have been using this form since its first submission on September 3, 2017.  All Google sheets integrations have gone through successfully until today when one real submission did not filter into the integrated Google spread sheet as all the others did, and then I sent a Test submission and it did not sync with Google sheets either.  I have no screenshot to offer.

I checked your form revision history and seems like you made changes to your form a week ago.

Kindly note than any changes made to the form will break the integration and you will need to re-integrate google SpreadSheet again.

I would recommend you delete the integration and add it again to your form, then test if data is sent to the Google Sheet Document.
