I have only 1 form and have added mailchimp into the form. I have been able to map most of the details over but cant get the city and zip code to map over. I have been trying everything and no luck.

How about using Address tool under "Quick Tools" and then on your mailchimp form, use address tool.
Example, on your JotForm.
Then map them manually on your mailchimp integration wizard.

Thats how I had it first but if you look at your screen shot the state and city both say first name. I would take that the first name would get imported. Plus I dont have all of the form options, I have maybe 15 and it shows 5.

Sorry for that confusion, I intentionally not to map those fields except only the address field. But, here's the new screen:
I think city and state labels are designed for "complete address field" so try changing your field labels to something like "your state", "your city" just like my example above. After that, try to re-integrate your form. Be sure to remove first the integration then integrate it again. That might help.

Nope still not working, I have a screenshot. Here is a screenshot of the options I have. If you look there are only a few but the form has tons of them. Seems to me form maping to the integration wizard is off. I have unintegrated it and redid it a few times nothing at all. I have even now added the other Address field and still nothing.

Those fields will not display until you have them on your mailchimp, example if you have checkboxes available on your mailchimp, checkboxes will become available on the integration list. Anyway here's my screen:
Since you've break down the address field into separate fields, I'd suggest to rename them like "Your City" instead of "City", this should display on the list like in the screen below. I'd also suggest to clear your browser's cache after removing the integration then integrate it again.

I did read that in your other post and have also tried that too. No luck, trust me I have tried just about everything before I contact you. On mailchimp I only have email, name, City, State, Zip
I had just the address field and did not work, could only pull the address. Then it would be placed into the wrong feild on mailchimp
I Removed that and added each feild broke up and now I can get the zip code correct but sill not the city and state.
I then renamed them like you said and still nothing
Log into my account and try for yourself!!! Thanks for you help so far, just nothing working.

Kindly check your form. I've used my mailchimp key, I was able to map the two fields (city & state) successfully. Now, remove my integration, after that clear your page cache and then integrate your api key. That might help.

Alright, what every you did I lost some changes that I last did. I deleted and readded it back and still the same. Beause your API code worked I deleted the api code from mailchimp and got a new API and that worked!!!
