I was trying to input my email address to have the form sent to and I was thrown out and now my account reads "auto suspended". Can you please help? I hope I didnot lose all my forms......

As far as I understand, the username of your account is budman61.
That accound was autosuspended due to some forms were detected as phishing ones (some of fields were requesting personal information, such as "license", "credit card" and so on).
I have reviewed them and can't see anything suspicious at the moment. I can also advice to add a signature field to decrease a phishing score.
Your account will now be reactivated, but I encourage you to review the Terms-of-Use in order not to be autosuspended again.

You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
