How to have a form electronically signed?

    Hello, I am in need of a format that will allow me to ask for electronic signatures for an online waiver form ( three pages) giving permission to their student aged children to participate in my education program during or after school. I need to have it signed electronically and then filed in a way that I can track who has signed and who has not in a database. 

    Will your program do this for me?

    Yes, that would be possible.

    In the form that you will be creating, please add a signature widget.

    The submission data will be stored in the form's submissions page — How-to-view-Submissions

    Where you have the option to export it to PDF, CSV, Excel — How-to-Partially-Download-Form-Submissions-as-Excel-CSV-PDF

    Or you can also create a report that can also serve as your checklist — How-To-Create-Form-Reports-in-JotForm

    You can also set up email notifications that will be sent to your email address when your form is submitted and an email autoresponder that will be sent to your respondent's email address if they submit your form.
