Account Suspended

    Hello,

    We have recently received an email explaining that due to the fact we use Jotform to collect ssn's that we would need to add the "mandatory widget", "Terms of Use widget", and "Signature widget".  We would like to comply with your terms and requests but we are having trouble logging into the account due to suspension.  Can we lift the suspension so that I can get into the account and add the aforementioned widgets to our forms?  We love your service and would like to continue using Jotform.

    I checked your account and it seems your form Modern Family Law ADP Direct Deposit has a field that is requesting for Social security number which violates our -Terms-of-Use and that triggered the suspension.

    To lift the suspension, we need to remove the guilty field and after that you need to add the  widgets mentioned above to be able to use ask for SSN#.

    We will wait for your response.
