Forms won't load

    I can edit forms, but when I select "view form" I get an error message.

    Unfortunately, I could not recreate the issue. I can reach your form with it's direct URL that you have provided. 

    But, upon checking the form, I have noticed that it has 2 broken conditional statements.

    Can you kindly remove or fix them in order to see if it is the reason?
