I am working on a School Survey and the template seems to have come over so what messy. Would someone be able to advise if it needs to be reselected or if there is something wrong with the template that was selected. It is on the second page.

I checked the form you have provided and I was not able to replicate the issue. Here's a screen capture of my test:
I cleared your form cache on our server. Please check it again and if the issue still persists, let us know.

I am able to replicate the issue you reported.
I will escalate this issue to our developers and we will inform you via this thread once this issue is resolved.
