I have tested the form using the direct link and the issue does not happen, I can see the form is currently embedded using the iFrame code so you may try increasing the form's height, if the issue persists, please try embedding the form using the default embed code, this guide will help you getting it: https://www.jotform.com/help/34-Adding-a-Form-to-a-Web-Page 

If the problem persists, please keep the default embedded code and let us  know.
