    How do we add an "Other" field to this where someone can fill in their own? For example, rating various applications where not all possible applications are in the matrix

    Hi,

    You can use a radio button, as you would be able to include a text box where users could type their own entries.

    Pretty sure JotForm will add 'Allow Other' option on dropdowns pretty soon.

    Also creating a conditional logic would be another option.
