Does jotform HIPAA compliant and offer data protection?

Have been searching for a way to allow patients to complete health history questionnaire before coming to the office for consultation, to save time.

Do you have other medical offices using your service for this purpose? Is it secure and HIPPA (Health Insurance Portability and Accountability Act) compliant so that the information patients provide is protected and private?
