Why is my account suspended?

    I am trying to find out why my account has been suspended and how to get if back to normal. Can someone please help? This is my second message.

    I checked the email address (booktravel@sweettstravelservices.com) and I did not see any JotForm account associated with it.

    Please provide any of the following so we can investigate it further:

    - Form link

    - The website where the form is embedded

    - Account username

    - The correct email address registered to the Account

    That is very strange. Our username is SweetTsTravelService. The email address registered to the account is booktravel@sweettstravelservices.com. We had the form on our website (www.sweettstravelservices.com), but removed it due to the error messages we kept getting.

    The account/username SweetTsTravelService  was already suspended due to violation to the Terms of Use.

    Scams. You agree that you will not use the JotForm Service or any form(s) you create using the JotForm Service for scamming purposes to attract, lure, or illegally obtain payment of any sort from others by unjustifiable means such as posting a jotform as a money-making program on a classified ads site as an example of that. Any account reported or found doing so will be immediately suspended.

    Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Collecting Sensitive Information
    . You may not use the JotForm Service to collect certain types of sensitive information, including but not limited to, credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm, Inc. including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

    The form was flagged as phishing form for capturing credit card account data. We do not allow such form used.

    Should you require payments using credit cards on your form/s, you should use Payment integration instead.

    Okay, so how do we get the account reactivated so we can take that off of the form? We would still like to use the form for our business without the credit card part.
