I have been testing your form and the signature box is allowing to sign, please see these screenshots:
- Firefox.
- Safari:
May you please provide us more details about the issue?

ios - iphone operating system
e-signature is problematic (can't sign). I tested smooth signature and it works just fine. I'll have to switch up the widgets.

Apologies for the inconveniences this caused.
I have just tested the signature box on my end, as far as I can see it works on an iPhone emulator using Safari, please see this screenshot:
The device I used was an iPhone 6S Plus, which device and iOS version are you using?
This may be related to the iOS version too, but we will further check this on our end based on the device you're using too.

That's because I updated the form with the Smooth Signature widget instead of E-Signature. Smooth works better, and is more compatible across devices and browsers.

The eSignature widget is working to my end. I tested the form using iPhone 6+ IOS 11.

Anyways, you can keep the Smooth Signature if that works for you. But if something will go wrong again, please update this thread or simply create a new one.
