I created a free jot form and when I copied the link to Facebook, I did a trial run. When I filled in the first field, all other fields filled in with the same information.

It is most likely the browser cache issue. The data you previously entered is saved in your browser cache and when you fill the form again it is offering you the option to fill the form with the same data. You can disable the Autofill inside of your browser and test the form again.
If the issue still persists, provide us the URL of the Facebook page where you added the form, so that we can test it.
