The product item is not appearing on the order form?

When I test and submit an order only the quantity appears but not the product. I have everything set to visible.  Am I missing something?

It seems as though your products have been set up using spinner fields.  Most of the fields have been labelled quantity:

If you want to create products but not collect payment, I would recommend using a purchase order field and setting up your products in the purchase order field:
