Show headers and text: Contract agreement

My form is actually an Agreement Contract.  I have name, date, and signature field at the bottom of the agreement text.

All the data and signature is captured just fine but the output is only the data fields and signature. 

Is there a way to save the entire form so I have a completed and signed document?

Figured it out.  There is a small Settings gear at the top right of the Submissions screen.  Check box that says "Show headers and texts"  and you can save output as a pdf.
