Why am I suspended?

    Your account got suspended because of the "2018 Client Questionnaire" form is asking for login information. 


    As per our terms, collecting login information is not allowed here in JotForm. We need to remove the guilty fields so we can re-activate your account back to good status. If that is okay with you, please let us know.

    We have deleted the guilty fields on the forms mentioned above and unsuspended your account. You can now log in.
