Can I create a confidential client intake form?

Hi, there,

I would like to create an intake form for transformational coaching clients that includes a lot of sensitive information on their family, issues, goals, etc.

I do not have a web site up yet but would like a way for clients to quickly and easily get me this information that I could then download on my own computer (I would send them an email through JotForm?).

To start I would like to use the free version of JotForm since I am just starting out and am not sure what's possible with JotForm yet.

Could you please tell me if what I'm wanting to do is something you offer and which template I would use to create such an intake form?
