How do I create a report that looks like the actual survey

I'm trying to create a report for a client, and when I submit the answers, it just shows the answers and leaves out the text and images that are on the survey.

I've read the FAQ's on this subject already and they didn't solve my problem:

Please advise, thank you. I'm not including a link or an image to the survey as it's copyrighted information.
