How to convert PDF forms into online forms?

    I've been all through the forum and found some good answers, but most are outdated.  So I apologize in advance if the answer I need is on the forum, but I couldn't find it.  

    I'm trying to create a simple 5 page new hire form that can be filled out online then sent as a PDF to me.  2 forms I can create using Jotform, but the other 3 are Gov't forms that I have the PDF for.  I would either like to create fields that are then mapped to the PDF or some how import the PDF as part of the new hire forms.  If someone can directly me in the correct direction that works, I'll be good to go.  Thanks.

    To my understanding, you have 3 Government Forms that are PDFs, is that correct? And you would like to have those forms converted into online forms.

    We can help you in creating your form(s). But in order for us to do so, can you give us more details, please?

    If you can share with us the PDF forms that you are trying to convert into online forms, that would be great. You can upload the PDFs to any file storage service online and provide us the public share key after so we can check it. You can also just include a screenshot of the PDF forms here in the support forum — How-to-Post-Screenshots-to-Our-Support-Forum

    If, in any case, I have misunderstood your concern, please let u know.

    Thanks.  Here are the 3 PDF's.  The instructional pages can be removed when we get that point.

    It is possible to convert the fillable PDF forms to online forms from the following URL.

    However, as I check the PDF forms provided 2 of them are having multiple columns which might cause issues in converting the PDF file due to the limitations. Please give it a try and see if that works. Also, creating a new form with JotForm is easy by using the intuitive drag and drop method of fields to the fields. You may also check for the templates shared by our users that might fit your needs.

    I keep getting this error when I try to import a standard PDF.

    As mentioned in our earlier post, the issue might occur due to the multiple columns and complexity of the PDF file. If you are trying to upload a different PDF file, could you share the PDF file to take a look?
