You can customize the fields from their properties section as seen in the below screenshot:

I could be misunderstood your question. If so, please can you clarify your question more detailed?

When you link it to mailchimp via integrations, you are asked if you want to send an opt in notice. I would like to do that but i would like to tailor the actual opt in email asking permission to email them. The standard one is very basic. Thanks

So it is something that has to be done on Mailchimps side, it requires paid account and you would have to have some coding skills in order to make it work.
