Payment Error: Payment was unsuccessful.

People are having trouble paying by cc with this form.  I've attached a screenshot and one person's account of what happened.  what would cause this?

"I’ve made sure to fill in every field (I think).  Have tried both visa and amex. Definitely not a credit card problem.  It rejects it immediately before it seems to even process the card.  Have any payments gone through?"

Can you please connect Payment Integration in your form with Stripe?

ps.  why would this have disconnected?

I'm afraid that we don't know the exact cause of this issue, probable cause could be that there was certain change/update in how the integration works, which could cause the break.
