Our Salesforce password was reset - so I went to update the integrations on the various forms. The Salesforce integration is authenticated, I match my fields and complete.
When I go back to the form, the integration is removed (invalid). So I repeat the steps and complete the integration. Its saved again, but when I go back to the form its removed again.

As I check your last edited JotForm, I see that the Salesforce is connected and seems to be working normally.
Could you try logging into a private/incognito window or a separate browser to see if the integration is working fine? Let us know if you need any further assistance. We will be happy to help.
