How do I create a form to embed on our website?

    Hello,


    I am trying to add a form on our website to allow people to refer a friend to our company.

    The form would include the person's name, phone number, email address. AND also would include their friend's name, phone number, and email address.

    Then a button that says submit. Which would then send the info over to my email address and one other email.  

    Is this possible? Also, what would it cost? I am not sure which cost structure we would need for this. We don't expect more than 25 submissions a month through this form. Thank you!

    Most of the fields you listed can be added to forms with simple drag and drop.  Here is more information on creating forms:
