Cannot remember my username

Jotform is so succefful for me I have not needed to login for over a year as the repsonders and email stuff is perfect.

So i have forgot my email or username i used, is there anywhere to reteierve it from one of my forms?

The form you provided belongs to the account username (ginvodkaco) and it is registered to the email address (contact@spiritkiosk.com).
