Hello,
My form is designed to collect information from someone making a request to a review board for approval. They need to submit their information no later than 6 days prior to the next review board meeting.
I want to be able to show 2 dates at the top of the form: the next meeting date and 6 days prior to that date (deadline for submitting to the next meeting date). Is there a way to do that?

As far as I understood your requirements, I can suggest to add 2 Date Picker fields to the top of your form. You may set the custom dates showed afterwards:
Setting those fields to "Read Only" can also be suitable for your purpose. This option will not allow respondents to adjust dates shown:
In case you also need to disable your form on a certain date, please go to your form's Settings and choose a Form Status "Disable on date":
Hope this information helps! But get back to us if you need any other assistance with your JotForms.
