I am having issues logging in

I verified my email, now when I try to log in it keeps giving me the error, Auto suspended.

I have checked your account and see that it is suspended due to the form http://www.jotform.us/form/80715917984167 which is asking for Instagram username and password. Please note that collecting sensitive information such as passwords, credit card information, etc is not allowed with JotForm. Such forms are considered as phishing forms and the associated accounts are automatically suspended. Please refer to our terms of usage for more information on phishing forms. I have now removed the guilty fields from the form and activated your account. Please refrain from adding such fields to the forms to avoid any further suspension on your account.
