We tried to login but we get an account suspended message

    Hello dear Jotform Team,


    since a few days our Form doesnt show up on our website anymore. We tried to log in but that also doesnt work. *Suspended* 

    What is happening? This Form is part of a Facebook Campaign. So please help as soon as possible.

    You have been suspended due to the driver license field in the form. This is a kind of sensitive data which we do not allow under our Terms of Use (https://www.jotform.com/help/8-Terms-of-Use).

    We understand that you are doing a trucking business, and you require this. For us to allow the form, we suggest doing the following changes:

    1. Add terms and agreement for users to agree to provide their license details.
