    Why is the form truncated on my web page?

    I built the form, added to webpage using the embed for Wordpress code.

    All the fields displayed, I was able to complete a test form.  Then I  added simple instruction text to the page, and now the form is truncated.  I only displays the first 3 or 4 fields on the page. 

    I have deleted and re-embedded the code, but still no luck. 

    Can someone please tell me what I did and how to undo it.  I really need to get this project finished tonight.

    then followed instructions and created a post

    As you can see, I have the same issue with both.

    Update:  I just realized that there was old text below the form when I submitted the test.  But I deleted that text as it was just the text of the form for people to email their signup info.  So... I went back in and added some lines of text below the script.  The form now displays.   HMM?? That sounds like a but.  So, now the question becomes, how do I display my complete form, without the extral lines of uesless text?

    Please try to clear your page cache. If the problem still exist on your end, please reembed your form using this methods;
