Why my form is disabled?

need help enabling my form

You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm, Inc. including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

You're asking for #Driver license at your form 80708680449162 "Selling My Item(s)".

In order to reactivate your account (and to enable your forms), we encourage you to read the Terms-of-Use. The offending field needs to be deleted also.
