I tried to login but I am getting a message saying "Account Suspended." What's the problem?

    Your account (anthonybaxter) is currently suspended. This form (Whole Systems Learning: TRIBE Program Intake and Consent Form) triggered the suspension. It has a field that is asking for Social Security Numbers.

    Collecting sensitive information like login credentials, SSN, driver's license are not allowed here in JotForm. Please check our Terms of Use.

    We need to delete the guilty field so that we can reactivate your account. If that is okay with you, please let us know. However, if you really need to collect this information from the user, then you need to add the following to the form:

    1. An agreement stating the usage of sensitive information. You can use the Short Scrollable Terms widget or Terms & Conditions widget.

    2. Digital signature field. You can use the E-Signature widget or the Smooth Signature widget.

    Dear JotForm Representative:

    Please delete the field that triggered the account suspension.  Once my access is restored, I will undertake the steps you recommend to collect sensitive information.  Our program provides support for foster care and adjudicated youth to succeed in education and careers. To that end and to satisfy our grant requirements, we need to collect sensitive data.

    I appreciate you assistance in this matter and look forward to continuing and expanding our use of JotForm services.

    I have already removed the field mentioned by my colleague above from this form Whole Systems Learning: TRIBE Program Intake and Consent Form
