Embedded form is not submitting, it says "please wait" and nothing happens.

but when I try it with sample, it works...

If the form submits fine from its direct link, but it doesn't from your site, then the embedding code could be conflicting with your sites code, please try embedding your form with the Iframe code: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code 

However, before you embed it, paste it in a text editor such as WordPad, and you will notice two parts:
