JotForm Account got Suspended

    I got an email that my JotForm account got suspended due to the violation of policy. I would like your company to review this decision. My JotForm is intended for my school project which will be submitted as an exam assignment for a fake online visual art venue booking system. The reason why I asked for the username and password is because that is one of the requirements for my assignment, which asks users to type in their password as a login system. All personal information would not be collected as this JotForm will be only sent to my professor for marking. Please resolve this issue quickly as I have put in a lot of effort for this. For more information, I could provide my professor's email for validation.

    I checked and your account s131109 is active. I saw that my colleague Aury activated your account, so I assume that the issue was resolved via an email.

You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
