Why my form is not working consistently?

Hi,

I created a camper application to upload to our website. I went in through my phone and computer and it worked perfectly. About half the people who try can get it to work. The other half either cannot enter an address or cannot submit it once completed. I cannot figure out what else to try because it is working on all my devices. 

I tested your form but I wasn't able to replicate any issue, the form filled and submitted fine:
