Why is my account suspended?

Bom dia,

Estou sendo suspenso a todo momento sem infringir as regras da JotForm, gostaria de saber o que está acontecendo pois se esta ferramenta me atender, eu pretendo ser mais um assinante, peço que vejam com calma por favor, obrigado.

I have checked your account and you have been suspended because of the login fields you have on your form "CURSO´S - AVELINO´S", here  is a screenshot of the guilty fields:
