Multiple choice option won't submit the form

I have a question on my form that requires a YES or NO.  If you select YES, it throws an alert that says "Select this checkbox" and indicates the NO.  

It will not allow the form to send with the YES selected.

I cloned your form and I can't seem to replicate the issue.

As I check your website, it seems there are errors on its js.

May I suggest embedding your form using the IFrame code.

Also, kindly not include the <Script></script> part of the IFrame.
