Why doesn't the header info print on my form submisison?

Hi

I have included header info on my form and the header is not printing on the submission PDF.  I really want it to print on the PDF!!!!  Please see page 2 of the form, link provided below

For you to print form submissions with headers and texts in it you will need to enable Text and headers in your submission page as shown in the image illustration below

​So too much is printing. I only want the header and not the instructions
to the reporter. Is there a way to achieve that?
Please advise.

Unfortunately there is no option of hiding the other texts and displaying the Form headers. You will need to either display both or none of the headers and texts.
