I entered a few inventory widgets in order for the form filler to enter a number of samples they are requesting however the widget question and answer does not show up on the submission email. Any idea why?

I have been testing your form and a cloned version of it, I can see the issue happens due to the images you have added, they are not in a public URL and if you load them separately you will be asked for login credentials:
Please either remove the images or load them from public URLs, this way you avoid the conflict with the widgets.
