    Not all form fields are showing when the form is submitted.

    I have checked your email notification and it seems that the field ID specified for the notification doesn't match the ones on your form.

    I would suggest you to remove the current notification setup and re-create it. 

    1. Open the form in the form editor

    2. Click on "Setup & Share" tab on the top toolbar

    3. Click on "Email Alerts"

    4. Click on "Delete Email"

    5. Click on "Email Alerts" again

    6. Select "Add New Email"

    7. Select "Notification Email", then click Next

    Then complete the wizard.
