I often fill out a form with a customer on the phone and suddenly and unexpectedly, the form disappears with all the data. It must then start all over again to the great displeasure of the customer.
How can these situations not happen again?

I recommend using the Autofill feature of the form.
User guide: How to Enable Auto Fill on Forms
This feature will allow temporarily saving the user's data on the browser they were using during the session.
So even if the form/browser was accidentally closed, when they load the form again the previous filled in data will be restored.
