Jotform request we reset password, cant log in. cant find templetes? Please advise.

I have checked your account RLHARRISON and it was suspended due to phishing suspicious, the form that triggered the alert is http://www.jotform.us/form/61633479499169 due to the SSN and bank account fields that you have there, please note that asking for such information goes against our terms. 

Asking for bank account numbers is not allowed on JotForm, SSN numbers can be asked only if you add any sort of agreement as well as a signature field to your form.

I have removed the bank account fields from your form in order to reactivate your account, please add the suggested elements, here are some widgets that will help you with that:
