Form is not working

I created a form and when I checked my website and clicked on the button to bring up the form the link said my form was no longer available. 

What happened to my form??  I made sure to save my form. I checked it multiple times the day I created it and it worked just fine. 

Could you please provide the form link so we can investigate it further?

Also, please provide the website link where the form is embedded so we can check it too.
