When I initially revert to 3/19's version (yesterday/Monday), it looks fine initially. When I open to edit, the logo disappears, the form looks zoomed out, and it is not at all accurate to ANY version.We wanted to revert because the date/time fields were not fitting within the form width and expanded extensively horizontally. That is another issue we have not been able to identify and correct. This is growing increasingly aggravating.Page URL:

I have tested the logo problem. You might have deleted your logo before and it might not have reflected the revision history.
I was able to view your form without zoom out effect as seen below:

Hi, gizem. We did not delete the logo. It was showing in Tuesday's live version and in Monday's - as well as all the way to last week. This form is used daily and we would have noticed if it was deleted. The logo only disappears when we revert, go to form list, then click the form to edit the reverted version. The form also becomes much more narrow and randomly assigns inexplicable round corners to fields.

To me, it seems an update was made to the form designer that affected our CSS, which we use on multiple forms created from the same template as this one.

I inspected your form and it currently does not look like the form on the screenshot my colleague previously provided.
Should the form look like this?
I cloned your form and I was able to replicate the issue you reported. It seems that the form styles are reset when the form is reverted to the previous version. For example, I saw that the width of your form is 975px. When the form is reverted to any other version the width is changed to 690px. The logo is also removed. All changes are visible when the form Builder is closed and re-opened.
I will report this issue to our developers and we will inform you via this thread once this issue is resolved.

Yes, the width should be 975px. We never deleted the banner nor adjusted the width - something in the system seems to be overwriting and disrupting our styling. Much appreciated!
