How to share form on tumblr

Hi there, I'm trying to put our submission form on tumblr as a static home page type post. I have followed the instructions for publishing on Tumblr, but can't seem to get it to work (the post just shows up as empty).

I tried the method above and seems not to work also on my end. However I tried the option f sharing a form as a link. Kindly follow the screencast below on how to do this on your end.
