Form: How to create a form with ranking functionality?

I am looking to add a ranking questionnaire that would allow our users to rank the selections from highest to the lowest level of importance to the survey taker.  Does JoTForm support this type of request?  (example is shown in attached picture)

It does not need to look like the format below, just provide the same "ranking" functionality.

You can use the orderable list widget. You can drag and drop the options depending on the rank you want.
