Problem with Travel Request Form

Hi there, I am trying to send out the form and one of 2 things happen:

1.  when the customer click on the link it is asking them to create a form

or 

2.  they fill out the form but cannot submit it

2. They fill out the form but cannot submit it

I did a test submission using the form's direct link and I was able to submit it. After hitting the Submit button, it redirected me to the Thank You Page.
