I cannot log in, it says my account is suspended.

    I have reset my password multiple times, and it says Login failed - AUTOSUSPENDED


    This is SOOO frustrating.

    I am trying to create a new client form. I don't need anything with a SS# on it. 


    I initially chose a healthcare form because it was the closest to what I need. Then i decided to make my own, please help.

    Your account (TeaganJ) is currently suspended. Our phishing detector tool automatically suspended your account. 

    This form (Alternative Medicine-Patient Intake form) triggered the suspension. The form has a field that is asking for SSN (Social Security Number). Please take note that collecting sensitive information is not allowed here in JotForm. I suggest you check our terms of use.

    I have now removed it from the form. Your account is now activated.

    If you really need to collect this information from the user, then you need to add the following to the form:

    1. An agreement stating the usage of sensitive information. You can use the Short Scrollable Terms widget or Terms & Conditions widget.

    2. Digital signature field. You can use the E-Signature widget or the Smooth Signature widget.

    Hello Jan,
    The alternative Medicine Intake form should be removed or the SS# part
    removed, as there is no disclaimer, hey if you pick this template, we WILL
    lock you out of your account.
    I just want to create a template for my clients. I am a Holistic Health
    Educator.
    Otherwise the template is useless. I was using it to see if I can remove
    some of the things I don't need, rather than building a template from
    Scratch...
