PDF Forms: The PDF that was produced has not been generated correctly.

Hi Guys,

The PDF that was produced has not been generated correctly. It is a survey type form and all the Question + Radio Buttons for responses just come out as blank text boxes on the PDF.

This is because you are using Scale Rating field instead of the single choice or input table field. Scale rating field is not be displayed in the PDF form, it is only displayed in a text box instead of the scale rating field.
