How to create a grid form?

    I would like to create a grid where the respondent fills in the row with their appropriate entity name and checks the box on the column that I create.

    Would you kindly provide more information on this? As per my understanding what you have requested may be achieved by using Name field and Input table fields as shown below.

    Yes, I would like users to enter the Names of the Organizations and the check the boxes that are appropriate.
    Board Member Partner Associate Employee ETC
    Name of Entity
    Name of Entity
    Name of Entity
    etc.
    User to complete the different organization names and click the column of their association.
    Can that be done??

    As I checked your form it seems you have a field similar to what you have asked. Does this field meet your requirements?
